Welcome to the Patrick Henry Local Schools Registration Page

Below you will find the FinalForms links for returning students as well as the necessary documents for incoming students.

If you are new to Patrick Henry School, and do/will reside within the School District limits, welcome! We recommend you start the process by filling out the New Student Registration Form below (one per student) and contact our Central Registrar, Sherrie Harris. She can be reached through our staff directory, link provided here, or call 419-274-3015 Opt. 5 for New Student Registration. We are thankful you have joined us and we looking forward to helping you through the registration process!

All returning students who live in the district need to fill out FinalForms annually for registration. FinalForms will save your information from year to year, making repeat registration easy! FinalForms registration opens in May and runs until the end of the school year.

Open enrolled students will also need to apply for open enrollment every year. Open enrollment closes on July 31. Open enrollment is completed online and the link will be found on our website when we are accepting open enrollment applications.

FinalForms is an online form management system. It allows parents, students, and staff to complete required and optional forms online (items like: Emergency Medical Forms, Acceptable Use Policies, Handbook Signoffs and other paper copies). FinalForms will replace paper copies that were typically sent home at the beginning of a school year and in many instances will make entering data easier for everyone.
